As Poplar celebrates a white Christmas, Nonnatus House receives an SOS call from a tiny mission hospital in the Eastern Cape that is understaffed, underfunded and threatened with closure. Transported to South Africa and facing some of their toughest challenges, the Nonnatus family digs deep to save Hope Clinic and make a difference to the lives of the people that use it. South Africa also proves a romantic backdrop for Reverend Tom Hereward and Barbara.
Season 5 of this critically acclaimed drama opened in the year 1961, and Poplar began to feel the winds of social change with the improvements in housing, sanitation and healthcare. But for the midwives and nuns of Nonnatus House, it has been business as usual, as they continued to care for the sick and bring new life into the world. The personal and professional lives of the nurses remained entwined, as events in their work have impacted on their personal choices.

Throughout the season, the midwives have encountered complex and challenging medical cases, ranging from an outbreak of typhoid to the increase in lung cancer, a teenage pregnancy and a mother who vanishes. But all are rocked when two babies with severe deformities were born under their care, several months apart from one another. As they struggled to understand why, they have has to find ways to deal with the resulting medical and emotional fallout.
